Presbyterian Heart Failure Telehealth Program Helps Patients Manage Their Conditions at Home
The Presbyterian Cardiovascular Institute's Heart Failure Telehealth Program helps patients develop self-management skills at home.

This three-month program is appropriate for:

  • Patients who are newly diagnosed and need additional education and support
  • Patients with existing mild or moderate (Class I-III) conditions

After a one-time visit to the Heart Failure Clinic, patients receive educational materials and a scale that transmits data to the Heart Failure Telehealth Program team. The team works with physicians to monitor conditions and help patients achieve and maintain healthy lifestyles. Team members include:

  • Nurse practitioner
  • Nurse case manager
  • Dietitian
  • Social worker
  • Resource specialist

Patients with severe cases of heart failure (Class IV) should be enrolled in the Presbyterian Heart Failure Program.
